Is the care recipient already receiving Medicaid Home Care Services (personal support) through CCSP, SOURCE, or ICWP?

In order to qualify as a paid family caregiver in the Structured Family Caregiving Program(SFC), the caregiver must meet the following requirements. Please read the following requirements carefully and click the checkbox on each line to show acknowledgement. (All 15 conditions must be met for caregiver to be eligible to receive stipend payments through the SFC program)

At least 18 years old
Live in same home with member (care recipient/participant)
Not Employed
Not Self-Employed
Not the parent (if the care recipient is a minor or unemancipated adult)
Not the member's conservator or legally appointed guardian
Not the Representative Payee (as established by the Social Security Admininstration)
Must pass a background check
Must log tasks and submit notes daily through SFC provider's secure electronic app or website
Must communicate pertinent information t member, Health Coach, and/or case manager
Must partake in SFC provider training on how to use the tools of a home care aide, which include blood pressure cuff, stethoscope, thermometer, gloves, gowns, aprons, shampoo boards and medical records.
Must compete 8 hours of training every year
Must speak with Health Coach at least once per month (by appointment)
Must meet Health Coach and Case Manager in person at the member's home, at least twice per year
Must notify the team when member is admitted to/discharged from the hospital or other care facilities
